Case Manager - Community Connections


About the role – Full Time Contract until 30/06/2028  

As a Case Manager in the Community Connections program, you will provide intensive short-term case management support to individuals aged 18–64. Using a strengths-based and trauma-informed approach, you will help participants build wellbeing, independence, and community connections by identifying goals, accessing services, and developing sustainable support networks.

You are a compassionate and motivated professional who is passionate about supporting people to achieve positive outcomes. With a strong commitment to person-centred and trauma-informed practice, you build trusting relationships, work collaboratively with stakeholders, and thrive in a dynamic community services environment.

You will also have the following skills and experience:

  • Experience in case management within the community services sector.
  • Strong understanding of trauma-informed and strengths-based practice.
  • Excellent communication and relationship-building skills.
  • Ability to engage effectively with diverse communities and stakeholders.
  • Strong organisational skills with the ability to manage competing priorities.
  • Experience maintaining accurate records and preparing case notes and reports.
